Al-Aqsa Community Clinic is a 501(c)(3) non-profit organization dedicated to serving the uninsured and those in need. All contributions made to ACC are tax deductible.

To All Patients
Please bring the following items to your first appointment:
- A W-2 Tax Form (from the previous year)
- Your last two pay stubs
- Verification of identity (Driver's License or Passport)
Now Offering Virtual Appointments
If you are scheduled to have a virtual appointment, you will receive a courtesy reminder call 2-4 days prior to your appointment to confirm. If the receptionist leaves you a message, we ask that you please return our call by 7pm on Friday, the day before your appointment. Failure to do so may result in the loss of your appointment.
