Acute Care

Provides care that addresses issues that appear suddenly and need more immediate treatment, for adults and children

Long-Term Care for Chronic Illness

Provides care that addresses long-term medical diagnoses (diabetes, high blood pressure, etc.), for adults and children.

Laboratory Services & Electrocardiograms (EKGs)

Through LabCorp, we are able to provide patients with lab requisition sheets in order to receive appropriate testing. We also provide in-house EKGs for our patients.

Referrals

We refer patients who are seeking additional specialty care at a more affordable cost through financial assistance applications.

Eligibility

The Yellow Card
Patients who qualify to be seen at our clinic will receive a yellow card. Yellow cards are required at each visit.

*Note: You must not qualify for public or private insurance (i.e. Medicaid, Medicare, NC Health Choice, Veterans' Benefits). Household income should be no more than 200% of the Federal poverty level.
